Home Health Care
Our home health care provides the kind of care you or your loved one need; in the comfort of your home environment. Sometimes a helping hand with home health care such as bathing and dressing is all it takes or, a companion, to prepare meals and do the daily chores. For a seriously ill patient, a qualified Registered Nurse will administer the highest and most efficient quality care. Whatever level of nursing care is necessary, our professional staff, including registered nurses, certified nursing assistants, homemakers, companions, etc. are trained to respond quickly to our patients’ condition, and concerns. All cases that require personal care are supervised by case managers and are provided in accordance to a care plan designed for the patient and signed by their primary care physician. Azi nursing group has built reputation for providing reliable, well-managed healthcare services and does not hesitate to make any changes in caregivers if need be. Our services are provided hourly, overnight and/or around the clock
Whenever you or a loved one needs nursing help, we are here. Our office is open seven days a week, 24 hours a day. You’ll be reassured to know that all our employees are carefully screened, including references, credentials, licensure and criminal backgrounds checks, and insured under AZI’s, workers compensation, and malpractice coverage. We follow all applicable state and federal laws and regulations. You can be confident you or your loved one will receive excellence care.
Specialized Nursing Care
When you call Azi, you will talk to a coordinator that cares about your needs. This coordinator will follow the patient with a personal approach and continuously be in contact with the patient and family to monitor their care and concerns. You can be confident you or your loved one will receive health care that meets a standard of excellence. We accept referrals directly from families, physicians, and other healthcare professionals. Azi provides a full spectrum of services in various areas.

Azi’s Approach

Before assigning professional nursing care to your loved one, our Case Manager first makes a home visit to evaluate the patient’s needs. A personalized care plan is then prepared. This incorporates the physician’s orders and the special needs of each individual patient. By matching the patient’s needs to the appropriate level of care, unnecessary costs are eliminated, and the patient gets precisely the care they require.
